Our Commitment to Patient Care
At Broadview Health PA, we are dedicated to providing the highest standard of care in sleep disorders, behavioral medicine, and psychiatric treatment. To ensure the best outcomes for every patient, we have outlined our office policies and procedures. Please read these carefully and sign where required.
1. Appointment Scheduling Policy
Patients must schedule appointments directly with Dr. Onafuye or other Broadview Health providers.
48-hour advance notice is required for cancellations.
Missed appointments or cancellations under 48 hours are billed $100.00, payable before future visits.
Broadview Health reserves the right to decline care at any time without explanation.
The first visit is an assessment only and does not guarantee ongoing treatment.

2. Prescription Refill Policy
Medications are prescribed to last until your next scheduled visit.
Missing or rescheduling an appointment may result in a denied refill.
Approved refills require a $25 processing fee.
No weekend refills and no early refills will be granted, including for lost or depleted prescriptions.

3. Forms and Documentation
Forms or letters requested outside of appointments incur a $100 fee and require 10 business days for completion.
Disability forms are only considered after a minimum of 3 months of treatment.
Completion of documents is at the discretion of the provider.
4. Provider Communication
Patient calls will be returned after all scheduled appointments, typically after 6:00 PM.
If the provider is unavailable, staff may return the call with instructions.
Some calls may take up to 24 hours to be returned.
5. Grounds for Termination of Care
Broadview Health reserves the right to discontinue treatment for:
Repeated missed appointments without notice.
Failure to pay for services.
Non-compliance with treatment plans.
Tampering with or refusing pharmacological screening.
Rude, aggressive, or inappropriate behavior toward staff or providers.

6. Payment and Financial Policy
Payment is due at the time of service, including co-pays and co-insurance.
Patients must contact their insurance provider to confirm coverage before appointments.
Medicaid patients must bring a valid Medicaid card to every appointment.
Returned checks incur a $25 fee, after which only cash will be accepted.
Balances over 60 days may be referred to collections, including additional legal or agency fees.
Patients authorize Broadview Health to contact them via any phone number provided regarding billing or medical matters.
